1. Acceptance of Terms

By accessing and using this website and the virtual medical consultation services provided by Dr. Farah Siam, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.

Dr. Farah Siam is a licensed pediatrician in Ontario, Canada. These terms govern your use of our website and all virtual medical services provided through this platform.

2. Medical Disclaimer

The information provided on this website and during virtual consultations is for educational and informational purposes only. It is not intended as a substitute for professional medical advice, diagnosis, or treatment.

• Virtual consultations do not constitute an in-person medical evaluation and may have limitations
• In emergencies, please contact emergency services (911) or visit an emergency department immediately
• Dr. Farah Siam will assess whether your condition is appropriate for virtual consultation at the time of booking
• Some conditions require in-person examination and will not be managed virtually
• Always consult with Dr. Farah Siam before making any decisions regarding your child's health

3. Virtual Consultation Services

Virtual consultations are provided through secure video conferencing platforms. By using these services, you acknowledge:

• You have a reliable internet connection and appropriate device (computer, tablet, or smartphone) with camera and microphone
• You are in a private location where patient confidentiality can be maintained
• All consultations will be conducted in accordance with Ontario Regulation 991 (Virtual Assessment and Treatment)
• Virtual consultations may be recorded for quality assurance purposes with your explicit consent
• Follow-up notes will be generated and stored securely in your medical record
• Virtual consultations may be interrupted due to technical issues; you will be contacted to reschedule

4. Appointments and Cancellations

Appointment Booking:

• Appointments must be booked through our online scheduling system or by phone
• You must provide accurate contact information and patient details
• Parents/guardians must be present during consultations with children
• A valid phone number is required for appointment reminders

Cancellations and Reschedules:

• Cancellations must be made at least 24 hours in advance
• Cancellations made with less than 24 hours notice may incur a cancellation fee
• No-show appointments (without prior cancellation) will be billed at the full consultation rate
• Technical issues on the patient's end do not exempt from cancellation policies

5. OHIP and Payment

Payment terms for services are as follows:

• All virtual consultations are private pay and do not qualify for OHIP billing
• Fees are clearly displayed during the booking process
• Payment is collected at the time of booking through secure payment processing
• Receipts and invoices are provided electronically
• Refunds for cancelled appointments are issued according to our cancellation policy
• If a consultation is cut short due to technical issues on Dr. Farah Siam's end, you will receive a refund or credit

Note: Parents/guardians may request receipts for submission to their private health insurance plans.

6. Referrals and Medical Records

If further testing, specialist consultation, or in-person follow-up is recommended:

• Dr. Farah Siam will provide referrals to appropriate specialists or family physicians as needed
• You have the right to request copies of your child's medical records at any time
• Medical records will be transferred to other healthcare providers upon written request, in accordance with Ontario privacy laws
• Records are maintained for a minimum of 7 years from the date of the last encounter
• There may be a nominal fee for copying and transferring records

7. Intellectual Property Rights

All content on this website, including text, images, videos, and educational materials, is the intellectual property of Dr. Farah Siam or used with permission. You may not:

• Copy, reproduce, or distribute content without explicit written permission
• Use our materials for commercial purposes
• Modify or create derivative works from our content
• Remove copyright or attribution notices

Educational materials provided during consultations are for your personal use only and should not be shared publicly without permission.

8. Privacy and Confidentiality

Your privacy is protected under:

• The Personal Health Information Protection Act (PHIPA) - Ontario's privacy law
• The Personal Information Protection and Electronic Documents Act (PIPEDA)
• Canada's Privacy Laws

All patient information, including medical records and consultation notes, is kept confidential and secure. Information will only be shared:

• With your explicit written consent
• As required by law (e.g., child protection services)
• For continuity of care with other healthcare providers you authorize
• With payment processors for billing purposes (no sensitive health information is shared)

9. Limitation of Liability

To the maximum extent permitted by law:

• Dr. Farah Siam is not liable for any indirect, incidental, or consequential damages resulting from the use of virtual consultations
• Services are provided on an "as-is" basis
• Dr. Farah Siam is not responsible for technical failures, internet outages, or platform unavailability
• Liability is limited to the fees paid for the consultation in question

However, this does not limit liability for:

• Death or personal injury caused by negligence
• Professional medical malpractice claims
• Violations of patient privacy rights

10. Governing Law and Jurisdiction

These Terms of Service are governed by and construed in accordance with the laws of the Province of Ontario, Canada, without regard to its conflict of law principles.

You agree to submit to the exclusive jurisdiction of the courts of Ontario for any disputes arising from these terms or the use of our services.
